Log Form Feedback Phase - Seeking Survivor Feedback

Only you can see your individual log.
We will only see responses that have been accumulated for 7 required questions and up to 4 follow-up questions.
If you answer ‘No’ to deciding not to report or to seek medical, mental health, or legal support, 1 follow-up question will appear for each ‘No’ answer asking you to select any barrier(s) you faced.
These follow-up questions are also required to submit your log.
There will be no identifying information in the responses we see.
Because we are protecting your privacy and security we can't recover lost links. This ensures your answers are anonymous.
You can choose to start a new log anytime.

We will update our privacy and security policy as the platform evolves.
We are protecting your privacy and security to ensure your answers are anonymous.
No Accounts / No Notifications - you can use the platform without creating an account, username, or sharing personal information. Your privacy is protected, and we will never contact you.
Data Security - all data you submit is protected using industry-standard HTTPS encryption. Any IP addresses or device information are not linked to feedback responses.
For security and operational purposes, some IP and device data may be logged temporarily by Cloudflare, our hosting service. This data is handled securely and is not linked to your identity or shared externally.
Log Form Feedback Phase
Text Boxes on Log Form - for the log form feedback phase, anything typed in the text boxes will be included in your feedback. Be mindful to avoid including names, locations, or identifying details during this phase. You won’t be able to return to the form once it is submitted
Tally - the log form for the log form feedback phase is being administered through Tally and they do not attach IP addresses to submissions.
Use of Data - only anonymous usability and feedback responses are being collected during the log form feedback phase. No sensitive survivor data will be collected.
