First Time Here
Log Form Feedback Phase - Seeking Survivor Feedback
First Time Here
We believe you.
You did nothing wrong.
It wasn't your fault.
You’re not alone.
You deserve to be heard.
You are brave for being here.
Logging lets you anonymously name any barriers you may have faced, i.e., not being believed, fear of being blamed, retaliation, or unfair treatment.
You’re in control.
This space is available whenever you feel ready.
We will not ask for your name.
You won’t need to create an account.
We will not contact you or be able to identify you.
You are the only one who will be able to see your anonymous log.
It will never be visible to anyone else.
It will never be used against you.
This platform does not notify your school, Title IX office, law enforcement, or any authority. It is not a formal report.
Logging here, whether you choose to report or not, gives you a place to safely record what happened, helping you feel more in control on your healing journey.
Healing is possible.
Logging here is safe, anonymous, secure, and institution-free.
Logging lets you anonymously name any barriers you may have faced, i.e., not being believed, fear of being blamed, retaliation, or unfair treatment.
Logging also gives you an opportunity to share your demographic information anonymously.
What happened to you matters. You deserve to be believed and supported, even if you never initiate a formal report.
The log form questions should take less than 5 minutes to complete.
To submit a log, these 6 questions are ‘required.’
1) How long ago did it happen?
2) Did you know your perpetrator?
3) Have you reported what happened?
4) Have you received medical care?
5) Have you received mental health support?
6) Have you received legal help?
If you answer ‘No’ to deciding not to report or to seek medical, mental health, or legal support, 1 follow-up question will appear for each ‘No’ answer asking you to select any barrier(s) you faced. These follow-up questions will be required to submit your log.
9 demographic questions will be optional, with a choice to select ‘Prefer Not to Say.’
7) Gender
8) Current age
9) Race
10) Sexual orientation
11) Student status
12) School type (undergrad, grad, etc.)
13) School size (number of students)
14) Region in United States
15) Disability status
There’s one final ‘required’ question.
16) Do you consent to share your optional demographic answers? (‘Yes’ or ‘No’)
After the questions, there will be a space for you to type anything you want. You will be able to return to your log later to read it, edit it, or add more. This will be private to you and never seen by anyone else.
You will receive a unique encrypted link after submitting your log.
Please save this link to return later.
